"Health at Home" for elderly patients with heart pathologies PDF Print E-mail

Coordinated by the Italian Research Centre “Consorzio Pisa Ricerche”, this European project aims at solving problems related to the provision of health services to elderly patients with chronic heart failure. To overcome this objective, CITIC, together with other 7 European participants, will develop a system with wearable sensor devices for self-monitoring of pathophysiological parameters and, at the same time, enable the medical staff to tele-monitor the situation.

The H@H Project will develop a new architecture for the socio-sanitary assistance services of the collective, including a database that permits collecting, transmitting and recording of Medical Data, under the specifications of European Countries and international standards, through a software system. It will be very helpful for visualizing the level of hazard of the situation, alerting, if necessary, the medical staff.
Following one of the main principles of the “Ambient Assisted Living” Program, the initiative counts with the direct participation of a end-user representatives in the consortium. These partners have a fundamental role in identifying the needs of end-users, and in improving and validating the developed tools.

From CITIC, we can declare that this research contributes to the efficacy and efficiency of the European Health Assistance. Elderly with heart failure will be able to recover at home, reducing their stay in hospital, which will produce a positive effect in the hospitalization costs of European countries.

Reality indicates that in European countries the number of elderly compared to total population is drastically increasing. This process has a consequence in the medical assistance demand with the objective of increasing independence. Moreover, as a result of ageing of the European society, the number of patients with chronic diseases has raised in last few years.
Counting on these previous dates, “Health@Home” project will face these challenges by developing a new type of assistance; it will be based in a multidisciplinary focus, generating a new European integrated model of sanitary assistance and information management for health and social work.
